To request services, please follow the link below to access our digital intake form. Providing thorough and accurate information on this form helps us determine whether we have a clinician available who can best meet your needs. Please note that submitting the intake form is not a guarantee of services. Our goal is to ensure that when we accept a new patient, we are able to provide the most appropriate and effective care possible. If we determine that we are unable to meet your needs, our front office will contact you with the reason and, when applicable, recommendations for next steps or alternative resources.

If you are requesting a neuropsychological evaluation, please note that your intake cannot be processed unless a referral from a physician, psychiatrist, or nurse practitioner—with justification for neuropsychological testing—has been received. Referrals may be sent to our office at 608-242-7153.
